IMPORTANT DUE DATES FOR HEALTH RECORDS INCOMING 6TH GRADE STUDENTS
- Completed physical exam, along with the TDAP and Meningococcal vaccines are due by OCTOBER 15TH, including health history (completed by parent/guardian) on top of physical page.
- Parent/Guardian will be notified if there are any other vaccine issues.
- Students cannot attend school starting on October 15th until all completed medical records are turned in.
- Appointments will not be accepted after October 15th.
- A 6th grade dental exam is due by May 1st.
NEW STUDENTS
Students new to our district have 4 weeks to turn in physicals and immunization records. They must be less than 1 year old and on an Illinois form. After 4 weeks, your student will not be able to attend school without these records.
7TH AND 8TH GRADE SPORTS PHYSICALS
If student would like to participate in school sports, they must have a completed sports physical the week before that particular sport starts. The sports physical is good for 13 months from the date of the exam.

2nd Grade Dental Exams
for the 2020-2021 school year
are due by May 1, 2021
Click on "Elementary Events" tab to the left for information about our DENTAL EVENT.
To qualify as a 2nd grade dental exam:
- The visit date on the form may be dated from November 15, 2019 through May 1, 2021. For any dental exam performed for your second grader during those dates, you may call your dentist and ask them to fax or email a "Proof of Dental Exam" form to us.
