New Student Registration

Registration packets for students new to Woodland District 50 are available at the Educational Support Center located at 1105 North Hunt Club Road.

Registration for students new to Woodland District 50 is conducted at the Educational Support Center located at 1105 North Hunt Club Road, Gurnee. Please make an appointment to register when you pick up your packet or call 847-596-5683.

You will need to provide one Document from Category A and one document from
Category B. You will also need to provide your child's Birth Certificate issued by the county, NOT the hospital issued birth record. In addition, we will also accept your child's passport.

Category A: Choose 1 document from the choices below:

  • Most recent tax bill and proof of payment (Homeowners)
  • Mortgage Papers (Homeowners)
  • Signed and Dated Lease with proof of payment and persons residing at the address listed (Renters)
  • Letter of residence from landlord in lieu of lease with proof of last two months payments (Renters)
  • Housing Letter (Military Personnel)
  • An agreement of sale of residential property located within the district boundaries between buyer and seller (New Residents)
  • Notarized Residency Affidavit (those living with relatives or others) within the district boundaries, refer to forms below.

Category B: Choose 1 document from the choices below:

  • Drivers License, Illinois ID Card, or Matricula Card with current address listed
  • Current gas, electric, water, cable or telephone bill (cell phone bills will NOT be accepted)
  • Current homeowners/renters insurance policy and premium payment receipt
  • Vehicle Registration
  • Voter Registration Card

NOTE: Families who are not renters or homeowners but are living with a district resident, will need to complete one of the following forms and have it notarized. This affidavit may be used as proof of residency. Woodland’s Police Liaison Officer may conduct a home visit to verify compliance with the Illinois School Residency Code.
