Free-Reduced Lunch If you have received an Eligibility Certificate from the Dept of Human Services, please submit the original letter to Woodland School Dist #50, Business Office, 1105 N Hunt Club Rd, Gurnee, IL 60031. No application is required if you provide us with the DHS letter. To apply for free or reduced price meals, use the Free-Reduced Lunch Program Application forms (see links below). Be sure to provide all required information or your application cannot be approved. Return the completed application to: Woodland School Dist #50 Forms for the 2008-09 school year are available in English and Spanish as PDF files and are linked below.
